Remote cardiac monitoring provides a new lease of life for Welsh patients
Two Welsh health boards and Life Sciences Hub Wales tested our remote monitoring technology. During the pilot programme one health board saw a 74% reduction in the time needed to optimise patients’ medications and a 19% drop in the number of outpatient appointments. Overall, 10% of patients avoided readmission to hospital and 82% of escalated cases were identified because clinicians saw data via the app.

How do we support NHS patients?
Patients on our digital virtual ward platform access a digital care plan that has been configured for their needs by NHS clinical teams so they can be monitored and cared for at home, rather than being in hospital or needing to attend face-to-face appointments. Huma’s technology helps put people in charge of their own health and healthcare, working together with their clinical teams, and supported by authoritative educational content. Fully 99.7% of patients find the Huma app useful.1
Patients can log their symptoms and their severities through the app which can help clinical teams identify signs of worsening health condition.
Patients are given a timeline outlining the tasks they need to complete and receive timely reminders.
Patients can use their phones and connected devices to capture and monitor health vitals safely and remotely.
We support patients with educational content about their symptoms, care and treatment drawn from trusted sources.
The app can provide validated (or bespoke) clinical questionnaires, for example the Oxford Knee Score, EQ-5D, Global Health Score and Promis CAT.
Patients can receive additional support from their clinical team through secure video and audio consultations alongside secure messaging.
Physiologic and other data can be collected passively from the phone’s sensors, via connected devices or by manual data entry.







How do we support NHS
healthcare professionals?
We help people return home sooner, under the care of their clinical teams to free up bed days and increase clinical capacity. We aid clinical decision making by providing NHS care teams with real-time visualisation of patient data through a web-based portal. The portal features automated flagging systems, trend visualisation and telemedicine for direct communication.
The portal can display information about individual patients and visualise data trends, prioritising patients according to logic designed by NHS clinical teams. Clinicians can add notes and collaborate with colleagues to ensure the patient receives the optimal care they need.
Huma's highly modular and super-flexible platform can be configured for any condition or patient cohort, enabling teams to manage multi-morbidity and reduce the number of point solutions. We are experienced in both acute, community and primary care – and we’re already working with a number of Integrated Care Systems.
Clinicians can quickly and securely message and/or speak to patients through video and audio calls (which can be scheduled) to ensure they are on track with their care plan and to check any irregularities flagged in their health records.

The NHS Trusts we work with
Huma's digital virtual ward platform is established across 20+ NHS primary, community and secondary care sites. Our library of configurable modules enables the same remote monitoring platform to be used for people with long-term conditions (e.g. heart failure, diabetes) and co-morbidities as well as across surgical pathways and through acute episodes such as COVID-19 / respiratory infection.
Supporting people with heart failure in Wales

We work with specialist heart failure nurses at Cwm Taf and Betsi Cadwaladr University Health Boards in Wales. In pioneering work that earned the nurses a Nurse of the Year award, we saw improved patient outcomes, experience and clinical efficiency for people with heart failure. Covid 19 Digital Virtual Wards in North West London

Huma worked closely with clinical leads across the NW London ICS to rapidly co-design a remote monitoring platform. The trial almost doubled clinical capacity and reduced clinicians’ time spent reviewing patients by over 42% and reduced hospital readmission rate by >30%5 . Supporting the NHS with elective recovery

We support the NHS with elective recovery, including for cardiac surgery with Royal Brompton and Harefield hospitals and orthopaedic surgery through a novel partnership with Smith+Nephew. We assist clinicians safely and efficiently monitor, care for and manage patients awaiting surgery. We also help identify who needs further intervention, offer timely educational content, questionnaires, symptom and activity tracking, and telemedicine. In one project, 88.2% of surgical patients flagged as deteriorating had their surgical date brought forward.3
